Baseline Testing (Concussion Management) Required by ALL Chiefs

All Chiefs players are required to complete the concussion baseline testing prior to the start of the season.  Please click on READ MORE for instructions on how to access and complete the annual testing.

In addition to concussion baseline testing, all players/parents must update the player medical information on the PRIVIT application.  Please click here: Elgin_Middlesex_Chiefs_Team_Registration_PRIVIT for instructions on how to access PRIVIT.  (NOTE: this is in ADDITION to concussion baseline testing)

Please notify your team trainer if you have any questions.

NOTE: Please ensure that the test is taken in a quiet room with no interruptions, with adult supervision (coach, parent, trainer, etc.) in a monitored and supervised setting. The test should be treated like an exam. Please remove yourself from any distractions that might affect your concentration. This program has a built in response device that picks up on the subtleties of response. If a player conducting the test is not responding at a 100%, the test will come up invalid, and you will have to re-take the test under supervision.

Step 2: Enter your Customer ID Code- 466DTUKP77

Step 3: Select your organization on the “Please Select Your Organization” drop-down menu

PLEASE SEARCH THROUGH THE ENTIRE LIST CAREFULLY.

 

For example, you will be searching for “Elgin Middlesex Chiefs and your year of birth/division”  YOU WILL NEED TO SCROLL DOWN the list to find it.

 

PLEASE READ CAREFULLY TO SELECT THE APPROPRIATE TEAM

Step 4: Click “Launch Baseline Test”

Step 5: Begin Test

Please set aside 25 minutes for completion. It’s best to print this page to begin testing
